1Physical ActivityPhysical activity can be defined as any movement of the body that requires energy expenditure. This includes any motion you do through the day excluding sitting still or lying down.

4Safety Belt UsageA seat belt is a vehicle safety device designed to secure the driver or a passenger of a vehicle against harmful movement that may result during a collision or a sudden stop.
5Life SatisfactionLife satisfaction is the way in which people show their emotions, feelings and how they feel about their directions and options for the future.
6AnxietyAnxiety is a normal and often healthy emotion. However, when a person regularly feels disproportionate levels of anxiety, it might become a medical issue.

8Perception of General HealthPerception of general health refers to the perception of a person's health in general. Health means not only the absence of disease or injury but also physical, mental and social well being
9Blood PressureBlood pressure is related to the force and rate of the heartbeat and the diameter and elasticity of the arterial walls.
10CholesterolCholesterol is a type of fat found in your blood. Your liver makes cholesterol for your body. You also can get cholesterol from the foods you eat. Meat, fish, eggs, butter, cheese, and milk all have cholesterol.
11Blood GlucoseThe blood glucose level is the amount of glucose in the blood. Glucose is a sugar that comes from the foods we eat, and it's also formed and stored inside the body.
12Hemoglobin A1c levelThe A1C test is a common blood test used to diagnose type 1 and type 2 diabetes and to monitor how well you're managing your diabetes. The A1C test goes by many other names, including glycated hemoglobin, glycosylated hemoglobin, hemoglobin A1C and HbA1c.
13BMIBMI is a screening for weight, for example underweight, normal or healthy weight, overweight, and obesity (calculated based on weight & height values).

The Health Risk Assessment helps identify risks to an individual’s health based on 13 generic items that focus on perception of general health, level of stress and anxiety, unhealthy habits such as smoking, weight description via BMI, results to blood pressure, cholesterol, blood glucose and hemoglobin tests.

The table below shows the 13 items and the sense in which they are used in the HRA.
